Last time I came here through Canada and my passport was stamped in Canada. Should I put the US city I first entered as my POE or the Canadian city where my passport was actually stamped and my documents inspected?

You mean you entered via a PFI? The PFI is your place of Admission, not the city where you landed.

Mention the pre-clearance facility at whichever airport you boarded your flight from. The stamp should have the 3 letter acronym.
