Please help me. Do I Need affidavit for short /long name for filing I-485!!!

Hi folks,
I am palnning to apply I-485. I am facing following confusion.
I have name in passport: patel(last name) xxxxxxbhai(firstname) B.
name in birht cirtificate : patel(last name) xxxxxx(firstname) B.

there is no 'bhai' word in Birth Certificate. So i am confuse Do i need to get affidavit that both are same person or some otherway.....
If yes, then where can i do this?

your passport has
last name: Patel
First name: KannuBhai
middle:B
Full name is :KannuBhai B Patel

on your Birth certificate it is: Kannu B Patel

You need to get affidavit made and affidavit must state that:
KannuBhai B Patel and Kannu B Patel are same person and DOB of KannuBhai B Patel and Kannu B Patel is……. Place of Birth is….. Father name is….. Mother name is…

Your father and mother can also submit the affidavit for the above.

-------------------YES the can make affidavits before Notary on stamp papers. for YOUR affidavit the Notary can type on stamp paper and can send it to you for signature and you sign then send back and then again Notary will stamp/sign and send it to You or can give it to your parents(better the date on Your affidavit should be when you were in India ( they can get back dated stamp paper) with your affidavit copy you can also get it Published in State gazette and that will be PAKKA Proof for future. Lawyer or notary know how to do that.
