Please help.......H4 extension petition lost in mail

Dear friends,
Please help me with your valuable suggestions. My H1 and my wife's H4 extension was applied through premium processing and was approved within 6 days. But my lawyer never got the original approval notice of my wife's H4 extension. My wife was planning to travel to India next month and stamp her visa extension at the consulate. If she travels with her expired I-94 will there be any problems. We only have a courtesy copy of her H4 extension approval with us. Please advice how we can get another original approval notice and I-94 from USCIS.

You can request USCIS to send H4 duplicate approval notice. Because this notice is important which has renewed I 94.
You can do it your self for H4.

Your wife may be in India by now, but she doesn't need to have an H-4 extension approval notice to get her H-4 visa revalidated. All she needs is your original H-1B extension approval notice. Even if the Consulate wants to see her H-4 approval notice, a copy of the courtesy copy should suffice. She needed to extend her stay only because she was in the U.S. when her I-94 expired.
