NIW and Form I-140

Hello,

I am filling form I-140 to obtain NIW-physician. I am working at a VA. I would appreciate it if someone could help me with these questions :

1. Part 1. Since I am filling by myself, I should write my name and contact information at the VA (not organization+ its own contact information). Is it correct?

2. Part 3. My date of arrival in the US. Do they want my first date of arrival in the US (7 years ago) or my last date of entry (which is in 2006)?

3. Part 6. Number 7. Is this a permanent position? YES NO. Since I am a H1, I am considered as a temporary worker. My first idea was to write « no » (not permanent). However, I read on the « Self Help Series-Part 1: NIW for Physicians - Steps to do it yourself. » in this forum that it should be yes. I don’t understand since all people applying for a NIW are not permanent residents and can’t have accepted permanent jobs. Can someone tell me what I am missing here?

Write your name and your home adress in that part (leave organization blank)2.

Part 3. My date of arrival in the US. Do they want my first date of arrival in the US (7 years ago) or my last date of entry (which is in 2006)?

The date of arrival in the current I-94 in your hand (2006). In the cover letter mention your history since 7 years with copied of all old I-94s

Yes permanent position do not write no, either you are resident (temporary), doing research on grant (for certain time period) or even without work and sitting at home, just write permanent job.
