Need your expert help looking at a lease letter

Because of my frequent trips to Canada, I am preparing a series of documents to show I maintain my residence, and one of the things I am preparing is a lease letter that I will get notary for (per earlier advice from some of you), since I don't have a formal lease agreement for my current rental because the owner of the house is my ex-coworker/friend.

I would like your expert advice (Bobsymth, JohnnyCash, Jackolantern and everyone else who has been super helpful on this site) to tell me if the following template looks good to you and if you think there is a better wording for things since I am not an expert at these things. I just want to make sure I am bringing a piece of document that is correctly written.


To Whom It May Concern:

This is to confirm that ____________ has been renting two rooms from my 4-bedroom home property at [ address ] from ______ to present. His monthly rent is _________ and he always pays his rent on time.

If you have any questions, please feel free to contact me at ___________.

Looks good..you may want to add a sworn declaration sentence with your landlord and your signatures as well:

"I certify, under penalty of perjury under the laws of the United States of America, that this information is true and correct."

To make it more authoritative if you want, have it notarized (fill in the printed names ahead of time, but sign it in the presence of the notary, showing both your IDs).
