need help on I-90

I am going to file I-90 to get my GC corrected and kind of confused on "Date of Admission as an immigrant or Adjustment of Status" on Part 3. What does it mean? Should I put my GC approved date or the date I filed I-485?

** file with local USCIS, take 2 passport style photographs, copy of GC, passport
* if you have no stamp on passport get the stamp
update the experience of filing I-90 with local office becase poster in other thread also wrote that they give you copy of I-90 with STAMP( I have not read in past that they give copy of form I-90 with stamp)

Just adding:

In my wife's case, we didn't get a copy of I-90 but my wife's PP got stamped and she got a receipt as well. The officer took her original green card and not the copy which we brought.

I went to the San Jose office this morning. The officer reviewed my I-90 application, took my photographs (2), took my signature and fingerprint. He waived my application fee since it is USCIS administrative error. They put "unknown" for "Country of Birth" in my GC. I already had my pp stamped before. He said that stamp is still good. So no new stamp today. I was told the new card will arrive in 6-12 months. He took my original GC as well, but didn't give me any receipt for my application, nor the copy of my I-90. Is it normal?

JLCF485, what kind of receipt did your wife get for her I-90? Where did she file?
 
I recieved a post, and person went to file I-90 with local USCIS( person got GC approved after the interview with same local office earlier) his name was incorrect on the CARD. the local office did not take his form I-90 and told him that they need to get his file again transfred to local USCIS and they noted his phone # and told that they will call when file gets transfred.

The receipt is the proof that you filed I-90 and it shows the payment ($185 if it is your fault; $0 if it is a USCIS error). In my wife's case, they considered it a USCIS error because the officer is kind enough. She was also given a stamp on the passport to indicate that the name change. This ensures that the name is corrected during the I-90 process and at the same time extends the validity of the stamp for another year.
