Name Change issue? BUT NO NAME CHANGE

I submitted paper work for my mothers OCI application and I just checked the status and it says: "Name change Documentation missing".
My mother was naturalized in last june and her name is not changed at all. She didn't ask for a name change. Her name is still the same. She has her first name, middle name and last name on naturalization certificate. but for some reason she doesnt have her middle name on her US Passport. But every where else she does.
My father has the same situation and his application is sent to consulate by this third party company.
Please help.

It seems your Mother has First Name, Middle name and Last Name same as on Indian passport and on naturalization certificate. if that is true then who filled her US passport application? did she wrote the FULL name as on naturalization certificate? if her name was written correct on Passport application then you can call the DOS or US passport department and if mistake is from DOS side they should be able to correct the Name on US passport.

I just checked the application and it looks like on the application there is no middle name. So does that mean that she has to get a new passport and pay all the fees again?
Will the passport card work for OCI? its cheaper.

Thanks. I just called passport office and they said they will change it once I fill out D-5504. But my father has the same situation and his went through fine.

Canada and Mexico ARE international destinations. The passport card is only for land and sea travel to those two countries and some others in the Caribbean.
