I requested name change on my application (N-400).
Do I need to give the reason for name change or not required at the time of interview?
What is the procedure for court document?
When will the court document be issued?

You don't need to give a reason, nor will you be asked to give one. At the time of your interview, the IO will ask you to sign 2 copies of the name change petition form. One of the forms will be returned to you at the oath ceremony. It will have an embossed seal over the court clerk's signature and will serve as the official name change certificate. Your naturalization certificate will have your new name.

Depending on your DO, you may experience a delay in oath scheduling. If your DO primarily performs administrative oaths, a court oath will be scheduled for you, which may take anywhere from a few weeks to a few months. If your DO primarily performs judicial oaths (NYC, for example), you will not experience a delay.

There is no extra fee if the name change is done as part of the naturalization process. But if you do the name change separately you would have to pay a fee in most jurisdictions.
