N-400-Name for C

N400 I have a question to fill the name.
For A: I am going to fill my full name
For B: I am going to fill as on GC.

400 Part 1 for C
The Form asks: if you have used other names, provide them below.

I have not used other name AFTER getting the GC. But at the time of filing I-485 on form G325A I also wrote my present name as on GC and for other names used in past I also provided info and wrote my maiden name and same on I-485 form.

Do I need to write my maiden name for C of Part 1?

On N-400 part C, list any other names you ever used, whether it since being a GC or previous to that.

If your current legal name (part a) uses your maiden name and your GC (part b) uses your maiden name (or vice versa), then no need to list it again in part C. However, if current legal name and GC is the same (either maiden or married name), then you would list your maiden (or married) name in part C.

I filed I-485 with my married name.
My legal name on my passport is: Shikha XXX YYY
On my SSN it is: Shikha XXX YYY
On my GC it is: YYY XXX, Shikha (part of my middle name they made as Last name)
On my I-485 I fill up my form as: Last name: YYY. Middle name: XXX. First name: Shikha.

On N400:
For Part1 A I am going to write: Last name as: YYY, Given name as: Shikha: Middle name as: XXX.
For part B I am going to write: Family name/last name: YYY XXX: Given name as:Shikha (I will also explain on sheet that USCIS has added my middle name to Last name on GC and can send the copy of I-485 form,Copy of Marriage certificate and copy of AP that had correct name on it).

Part C: I will write my maiden name.

Do you see if the info I am going to fill is correct?

Yes, this is correct. You may also want to include copy of birth certificate as well.

I dont have Birth certificate.My birth was not registered with municipality and at the time of filing I-485 I got non-availability certificate and 2 affidavits from my parents. I have the non- availability certificate and Affidavits for my birth date.
I have the name change affidavit after marriage that was signed before Executive magistrate in India. That affidavit has my maiden name and that I got married on date………. to( my husband name) at (place) and I have changed name to Shikha XXX YYY and that I want to use my name Shikha XXX YYY for all purpose. That was also submitted to USCIS at the time of I-485 filing.

Should I show this affidavit and other birth date affidavit at the time of Interview or I should send copy of all with N400 and explain on cover letter for the discrepancy on GC.
My I-485 form that was submitted has correct info and copy of G325 that was sent to USCIS has also correct info and I have copy of all.

I have explained on Sheet that part of my middle name is written with my middle name on Permanent card.
I have attached with Sheet my I-485 receipt, copy of my EAD and AP with correct last name.
I will take marriage certificate and birth date affidavits at the time of interview.
Should I now mail the N-400?

I am almost at the same boat as you except that in my country the concept of a middle/last name is almost non existent and so they were used interchaneably and sometimes with just the initials. On the 485 it did not specifically ask for first name last name so it was easy to write one version of my name. Now I have just added the two other possible versions. I dont have any documents to prove it and so am just going to wait for the interview and explain it then.
