I got a "fix it" ticket and forgot about it, and haven't paid it. So, it went to the court and I made payment arrangements, and still am paying it. Does this count as a citation or a crime? My driver's license has been suspended also.

15. Have you ever committed a crime that you were NOT arrested for...
16. Have you ever been arrested, cited or detained by any law enforcement officer?...
17. Have you ever been charged with committing crime of offense?...
18. Have you ever been convicted of a crime...

These are the questions I'm worried about. Do I qualify for any of these. Please let me know.. Thanks..

If you had to go to court it's a citation, but not considered a crime.

Should I note that? What if I pay it? Should I still note it? Would this do any harm for me in the process?

Answer Yes to Q 16 and 17.
In theory answer Yes to Q 18 too because paying fine automatically result in a conviction but it is a traffic violation no one
cares so answer No to Q 18 to make yoruself look better But even if you answer Yes to Q 18, it will not make any differences
if the IO see conviction is a conviction of an ordinary traffic violation
