A cover letter is not required for filing N-400.
But you should still do one.
If an applicant is requesting a waiver of the english and civics exam, would he ask that someone else write a cover letter for him, or should it be from him? the reason i ask is because he obviously doesnt know how to write/read.
Unless he is represented by counsel, the letter should be from him.

I agree with the previous posters: the cover-letter, which is necessary in this case, should be signed by the applicant, even if somebody else actually helps the applicant write this letter.
Also, the letter would have to specify a rule under which the waiver is requested. Simply saying that someone cannot read/write is not sufficient. As far as I know, for the waiver of the English exam there are several formulas (50/20, 55/15, etc, meaning age over 50 with at least 20 years as an LPR etc). I think for the actual waiver of the civics requirement the only valid reason is a serious medical condition. On the other hand, applicants over the age of 65 can get a simplified civics test.
