Hello and thank to all.

I would like to clarify something about suporrt since I usually see the opposite case of my question.

H4 --> DV winer
H1B --> spouse of DV winer.

When I show or send the I-134 form, at time of interview (show) or at time of sending (send) the I485 plus package to Chicago?

How many I-134 forms are needed? If two forms is because one is for the winer (signed by H1B) and another one for H1B (signed by employer)?

I used to be on H4 (my husband was on H1B) and I was the winner. We only submit 1 I-134 (for the winner signed by H1B holder = spouse).

You submited just one saying H1B will supurrt the H4. You sent this I-134 with all the documents and medical, all at the same time,right?

In this I-134, the H1B (no US citizen or GC), will skip question 1 and answer 2 (I am -------- year of age blablabla----- since ------), correct?

What you would recomend of docuemnts to send... to Chicago?

you probably won't even need I-134, since you speak English and the spouse already has a job in the US. That pretty much shows you are not going to become a public charge. Plus, I-134 from a non-resident doesn't really mean much...
 
oh, and one more thing - I-134 is usually signed for the entire family, so the husband cannot sign an affidavit stating that he will support himself. Who will the US govt come after for funds if anyone in the family decides to become a public charge?
