medical results- got envelope + 2 forms

Guys,
After we did my wife's medical, the doctor's assistant gave me the sealed envelope + I693 (1 original) and also the vaccine supplement form (1 original). My understanding was.... both of those should be included in the sealed envelope too ???

Should I send the vaccine supplement seperately assuming that it wasn't sealed in the envelope ? Or should I send all 3 with my 485 package ? Has this happened to any1 ?

the doctor's office should put the vaccine supplement along with your I693 form in the sealed envelope. Double check with the doctor's office to see whether they put it in the sealed envelope. Otherwise, you need to make another trip to the doc's office to get the new sealed envelope or send the supplement along with the already sealed I-693. You just need to send the I693 and the vaccine supplement sealed in the envelope and keep the extra ones for your record.
 
my doctor made a mistake of putting the copies in the envelope and giving me the originals. I was still in the office, when I realized it, so I got them to switch it.
 
Out of interest how do you know what is inside the sealed envelope? You didn't open it did you? :o
 
no, but since I had the originals with the original signature in my hand without the envelope, I assumed the copies were in the envelope. I asked - the nurse opened the envelope - and voila - the copies were inside. She switched them, sealed the new envelope and apologized.

Sure enough, there's something inside the envelope :-)

I am guessing what's inside by looking at what's outside:

1. 693 form with original handwriting
2. vaccine supplement with handwriting

I would assume I get photocopies of the above (?)for my personal records.

I just called them up, the nurse said we have given u the 2 forms with original signature for your records, all others have been sealed in the envelope, rest assured. I guess...I will wait and see what the IO says in a few months LOL...

Has any1 tried using a blue light technique to peek inside the envelope ? just a JOKE !!! :-)

When I got my medical exam done, they gave me a sealed original and copies of everything that was in the envelope. As long as you the unsealed ones are copies and not originals, I would not worry about it.

...in my case, I have two originals outside the envelope.. LOL. .... i called the doc up ,assistant said they have other originals inside the envelope too. let's see what happens. maybe these guys just love passing out originals to all customers and save $ for the copier
