Medical Exam. Plese help.

Today I call an INS cert. Doctor. He said he need me bring a photo for my medical exam.

Can any one tell what kind of photo I need bring to Doctor? Can I use last year of photo for medical exam?

I plan to submit I485 in two or three months later, so, are the photo with medical report still valid because I485 photos are valid in 30 days of submiiting?

The form I693 does not mention any photo. As long as it is your photo you are taking along, I dont see any issue here...

Where can I get original I 'Form I-693' for medical examination? Doctor is not accepting electronic form and he doesn't have this form with him.

This doctor does not seem to be much familiar with INS check up. he is asking for a photo (heard for the first time, except he is asking for the ID), he does not have a form ...

why don't you try another one if possible
