Maiden name OR family name - DS230

I am filling DS230 for father and line 14 asks the MAIDEN or FAMILY name of spouse. Now, my mother's maiden name (last name) changed after marriage. All my mother documents before marriage contains her first and middle name but not last name (maiden name) - probably a common practice in india.
All her after marriage docs contain her update last name. Even the marriage they registered recently contains her updated last name (not maiden).
There are no docs that contain her last name before marriage even though she had one, since her father had one.

My confusion is what to fill in "MAIDEN or FAMILY NAME" for spouse in my father's DS230. Please advise.

Are they asking for maiden name or family name, or they are asking for maiden name and family name? If they are asking for maiden or family, If I were you, I'd use the family name, i.e. her current marriage name. This is because if there are any docs to be submitted, they most likely be current docs and not docs before she was married. And if they need any docs prior to her marriage, there should be a marriage certificate to justify the name change. If they need you to include her maiden name, then her first + middle name should do - I believe that's what her b/cert. has anyway.

Maiden name is your name before marriage. Maiden and family name is the same thing. Maiden does not mean last name, it means last name BEFORE marriage.

Each culture has its own way of interpreting maiden name and family name. In America, maiden name is your name before marriage. Family name is the the last name, i.e. the surname indicating the family to which you belong. Do google.
 
I do google, my mother was born and raised in India. Her maiden name was and is, her name before marriage. Her family name was the same as her maiden. I asked her first.

The point is that a maiden name can never change.
