Help With Employment letter

I need some help.

I am not sure about the content of the employment letter. I tried to find from my lawyer but he is not responding.
Can someone please help me by telling me the contents of the letter.

In form DS156.. there are certain coloumns like...
1.Name and Telephone Numbers of Person in U.S. Who You Will Be Staying With
or Visiting for Tourism or Business

2. Purpose of your trip...

3. Who Will Pay For Your Trip....

Can you suggest me some answers... I think they are relevant once you are coming from outside...

But in case of DS156.. do we have to write something....or just leave it as blank...

write whatever makes most sense or cross the column, don't leave anything blank.

e.g. purpose of trip
- Already in US
Who will support your trip?
- Self, already working in US
Where you will be living in US
- Give your current address

So write whatever makes sense, you can't go wrong.

For form I-797...

Should I send one page which is the Notice of Action.. or should I send the whole copy of petition...

You just need to send one page I-797 approval, petition is mostly they look at it if you get you visa stamped abroad. You can send original I-797 or the copy.
