JAXEN. EAD Extension with valid H1. Please Help

Guys, I need to Apply for my EAD extension ASAP. I need to know the following. Can anybody help ???

I'm planning not to use E-file. Right now I'm working on H1 and have sent my passport for revaliation. I'm applying on my own as my employer is refusing to apply for extension saying that I have valid H1. I'm filing for EAD extention as a backup to my H1 because you never know in this economy.

1) What documents we need to send
2) What is the exact fee. Do we need to make check payable to INS or BCIS ???
3) For question 16, what should be the answer.
Should be 'c9' . I do have a485- pending.
4) Should I need to send any signature card or Finger printing if I file by ordinary mail ???
5) Should I send a copy of my I-94 ???
6) Will they ask for I-140 approval notice ???. I don't have a copy and my employer may not give me one. My 140 was approved last year.
7) The fact that I have a Valid H1 makes any difference to my approval ???

Please let me know. I'm really confused with the fact that you have to take an appointment to give your signature and FP if you e-file ??. I don't want to e-file because it looks like it takes time and I don't have my passport to show my identity when giving FP.

I need to know, do I still have to give FP when not fileing through e-file.

1. U need copy of I485 receipt, check for 120 favor of BCIS, 2 ADIT style photos, a covering letter, filled I765 and copy of I94. No need to send signature cards, c09 is right.

6. Were you a concurrent filer? If you know the I140 receipt number, then you can check the status online and attach the printouts.
7. Nothing to do with H1.

--- No fingerprinting card needed, better send by mail because with efile again you have to visit application support center

---- If it is VSC they may not ask for I-140 approval notice but if you know the I-140 receipt 3 then go to BCIS Online punch your receipt # take the print out that shows the status of I-140 approved, attach that print out. If it is NSC also send color copy of your driving license, NSC is sending RFE for color copy of DL and first page of passport that has your photograph
7) The fact that I have a Valid H1 makes any difference to my approval ???
---- You can be on H1 and get EAD if I-485 pending, your H1 status will have no bad affect.
