Infopass problem

Hi Folks,
I tried to make appointment using infopass and it says that there are
no appointments available at this time and to try back at later time.
Is that normal ?
By the way, what are the documents I need to take for stamping ?
I guess it should include
1. Passport and approval notice
2. EAD and AP.
3. photos (what is the size ? & how many ?)
If any other document is required then please specify.
Gurus, please help.
