Immunization question

Hi,
I filed I-485 on April 19, 2002. I had done the medical at that time and all reports were fine. TD vaccine was not available at that time and the doctor mentioned that it was waived in the I-693. Today I got an RFE saying that I have not taken the required immunizations and need to take them and submit a new medical report.
MY QUESTIONS -
1) Is there a separate form to be filled if TD is waived?
2) Is TD vaccine available now?
3) Do I need to take Hepatitis B vaccine - (this is one of the items listed on I-693 under list of immunizations and my doctor left that line blank)
4) What is Hib (also under list of immunizations)

Maybe your doctor can generate a letter why this cannot be done now. Most of the doctors ask about the country from which the applicant came and then note that immunization was done years earlier...
