I-94 damaged

I am applying for my AOS application but my I-94 is missing a little piece from the upper left hand corner. So there 3 numbers missing. I am a Derivative asylee and eligible to apply for adj of Status. Do I need get a replacement I-94?

You should be okay. USCIS can dig up the info they need if it comes to that.

I am not sure what you mean. Please advise if I should apply for AOS without a replacement I-94? Should I leave the column blank on 1-485 for I-94 infor or put in whatever numbers left on I-94? Thanks

You don't know the I-94 number? Contact CBP to find out what it is.

Very easy. Go to port of entry airport(immigration dept.) and they will give you new one immediately.
