Submitting my H1B visa revalidation, but a bit confused at some of the instructions.

The instructions state to include: "The original or a photocopy of your current, valid Form I-797. A spouse/dependent applying separately from the principal alien must include the original or a photocopy of the principal alien's I-797."

Two questions:
- so, a photocopy is OK?
- and is this the whole (i.e., full page) "Notice of Action" document, not just the tear-off portion at the bottom?

Our immigration person gave me the original tear-off portion from the bottom, but a photocopy of the whole form. She's screwed up at least two other people's H1B applications in the past 2 years, so I'm a bit paranoid.

------ I would sent the original and keep copy with me
- and is this the whole (i.e., full page) "Notice of Action" document, not just the tear-off portion at the bottom?
--------- Full page and detach the I-94 from lower right side and attach it in your passport. In the cover letter explain what is attached with revalidation application.

*** for FedEx envelope( for sending and for return envelope) give the credit card that has expiry more than 4 months
