I-751 - Affidavits question?

I'm about ready to send the I-751 for my wife (I'm USC). Her GC expires 2/27/2010

I have about all documents ready except for this:

5. Affidavits sworn to or affirmed by at least two people who have known both of you since your conditional residence was granted and have personal knowledge of your marriage and relationship. (Such persons may be required to testify before an immigration officer as to the information contained in the affidavit.) The original affidavit must be submitted and also contain the following information regarding the person making the affidavit: his or her full name and address; date and place of birth; relationship to you or your spouse, if any; and full information and complete details explaining how the person acquired his or her knowledge. Affidavits must be supported by other types of evidence listed above.

In what format do those affidavits have to be?? Does anyone have an example? Furthermore, does it need to be notarized? The I-751 instructions say nothing about that..

Another question in regards to joint liabilities.. My wife is not on the mortages.. I owned the house that we live in since before the marriage and therefore everythigng is in my name.. We bought a 2nd property and I included a copy of the property records in both names BUT she is NOT on the mortage due to the reason that my mortage broker said that I should only take the mortage in my name as my wife had less than 2 years employment history and putting her on the mortage might prevent us from qualifying for the mortage on the 2nd house.. I hope that this won't be a problem..

Here's what I'm planning on sending:

2007 and 2008 1040's (filed jointly)
Car insurance cards with both names on all our 4 vehicles
I-551 front and back
Drivers Licenses showing same address.
Printout of Bank website showing joint ownership of checkings and savings account and a printout of all transactions from the last 60 days
Copy of United Healthcare card showing her being covered on my plan
Printout of Property appraisers website showing joint ownership of our 2nd property
Car title of the only car that we bought since getting married showing both names
Printout of our Expedia itenerary for our 2008 trip to Hawaii
Planning on including about 30 pictures
Of course the completed I-751 form with money order (I wonder if money order will be faster as they won't have to wait until it clears?)
