Will it be OK if doctor fills up the information in the internet
downloaded I-693 form and send it to INS?

My doctor refused to use the internet form

The Pittsburgh doctor I saw said the internet form was not valid.
He had copies of the original form and used that (its a multi-part,
multi-colored form).

Take a copy with you, but I am sure all civil surgeons have the
INS form--after all, they do this many times a week!

INS accepts all kinds of sources...

some INS approved doctors might be picky...
