I-130 question

I am am preparing to submit form I-130 for my mother via CP. I have a question regarding number 11 on the form.

1) My mother is a widow. The form asks for prior spouse (s) and the date the marriage ended. Should I include his name there and list his date of death as the end of marriage date? OR should I just include deceased in the date? Not sure what to do here.

2) Also, this is probably a stupid question, but I am currently married. So should I just fill in N/A for the line 11 that asks for my prior husband (s) and end date of marriage?

Just want to make sure the form is filled without any errors.

1) you should include his name and his date of death
2) if you had no prior marriages, "none" or "n/a" will do.
