I-129 question ragarding Individual Tax #

I am trying to file I-129, however, I can not find any document to indicate what is Individual Tax #, if I have federal Employer identification # and SSN, should I leave the individual Tax # blank?

if I have two passport(old and renewal), my I-94 stick to old passport, which passport number I should fill in I-129?

if TN was previous approved at POE, in I-129 part II 2, when I check b which is "continuation of previously approved employment without change with the same employer", I am asked to give the petition receipt number, what is the petition receipt number?

Part 3: Should Date of Last Arrival be the date at POE last time or the first date to move to US to work?
