Where is A# and where is I-94# ?

If you haven't been given an A# before through an immigrant visa proceeding or F-1, you do not have one. The I-94 number is in the top right, I believe.

The I-94 number comes from your I-94 card. A# are only assigned when you begin an immigrant process, not a non-immigrant status. Answer N/A for this.

Thanks. Is the I-94 number the 11 digit number under "Departure number" at the Top left part of the card ?
