Our office is in Phoenix. Here's the basic letter. You could probably remove the job duties. I added them in because I was going to use the same letter format as an AC21 letter. I made the INFOPASS appointment as "Questions about your case". Went there and got called up and I just said that I'm here to get an EAD and gave her my reciept notice. She calculated the number of days and said you have to come back on XX. I said my company is going to lay me off. She said you need a letter in that regard. I said, "Well....it just so happens I have one right here!!". She said ok and called CSC to confirm some info and ask some questions. She asked me to write up that "reason why you didn't file early enough" explanation. I just wrote, "mix up with lawyer", blah blah blah, and signed it. You may not have to go through that part though.
If that didn't work I was going to try using the "can't renew my driver's license" excuse. I did ask about that and they said it's not a good enough reason. If your dependant spouse or children need an EAD for driver's license renewal etc., you might consider bringing an EVL letter like yours if they're working or get someone to write up a "new job" offer for them.
