help needed to fill out i 485 form

Back in 2010 I had applied for asylum and the case was denied. Now i won dv lottery 2014 and got confusion in filing out some part of i 485.
1. In the form there is a space to fill 'A' file number. And what I remembered from my previous asylum case, the A # was given to me. So my question is, Do I need to write down that A# or leave it blank?

2. On the current status part, I fill F1 as I am student and I am in status though my asylum was denied. But what about on expires part? I am an undergraduate student and my anticipated graduate date is on may 2015. Also in my i 94 its written DS so what will be the expiration date?

1. Write the Alien number issued to you when you applied for asylum.

2. DS means 'Duration of Stay' so write DS for the expiration part of the question.

One last question, on I 485 form, PART 3, it asks for : have you ever applied for the permanent residence status in the US? which one i should check in for yer or no? As i have applied for asylum previously, was that count for the application for permanent residence?

Does that create any problem on my chances of getting GC upon putting A#? No, it shouldn't. It enables them pull your previous file and merge all the information they have on record for you. It saves them the hazzle of creating a new Alien number for you or you having multiple files with USCIS

One last question, on I 485 form, PART 3, it asks for : have you ever applied for the permanent residence status in the US? which one i should check in for yer or no? - Yes As i have applied for asylum previously, was that count for the application for permanent residence? Yes, it does and you have to be upfront about that in your new application, it's not the kind of thing one can hide and hope to get away with, they will still find out if you about it .
 
i got it now thanks much....Do you have any idea how much the medical cost for in Texas? I heard it varies from one city to another. and also I am planning to do my medical now and my case number is around AS00004***. when will i expect my interview? Do you think, the medical report will valid till my interview date if i do it right now or till the end of august?
 
No, I have no idea regarding medical cost. Yes, it varies from one doctor's office to another, not just from city to city. You might want to call and shop around from the list of approved civil surgeons in your area. Call the offices and ask upfront about the cost, some are willing to bill your medical insurance (if you have one), most are willing to collect cash only.

Medical exam in the US is valid for 1 year, however, I wouldn't rush into doing it now if I was in your shoes. It's hard to say what the CN progression rate might look like this year without seeing the first set of cut-off numbers applicable to DV 2014. So I would suggest you wait until the Sept VB comes out at least in order to have a better idea.
