help !I-797 Approval without I-94

Due to the fact that, I didn't have pay stubs from company A for a month, my new company B applied for new H1. I -797 approval has come without I-94 after 9 months of applying as it is taking more times these days. Now still I am working for Company B.

and then another mistake which has been done in the petition is, the consulate metnioned as "Toranto'", where as I am India.

1> Is there any way to get my new I-94
2>Is it possible for me to go to "Toranto" for stamping as it is not my home country.

Please help, my original I-94 is expiring pretty soon..

You can try to get the stamp in toronto. No harm . generally you have to get a new I94 immediately after approval of I797. You can even drive to the nearest port of entry and get the I94.
Toronto should not pose any problem.
