Has USCIS introduced a new form for AC-21

Gurus,

There is rumour that USCIS has come out with a form for filing AC-21, is it true. Also, is it better to send AC-21 letter by myself or use an attorney. Can you suggest a good attorney preferably in the bay area for filing AC-21, my current attoney is a dumbass and Sheila Murthy is too expensive.

If I need to send AC-21 letter by myself, please point me to the document formats and requirements. My case has been transferred to the San Francisco USCIS and will be coming up for adjudication soon.

------cove letter to invoke AC21+ new employer permanent job offer letter with same/similar job duties+ I-485 filing reciept copy. you can search "sample AC21 letter posted in this thread 3-4 months back)

----------- if the case has been transfered to Local office for interview you can send the letter to Local office and again at the time of Interview take new employer job offer letter when you go for interview becase they may not attach it with file.

Thanks Ginnu, do I need to send a copy of the Yates memo and make references to it in my AC21 letter. Some sample letters in this thread have done so.
