H4 withdrawal

My employer delayed filing for H1B extension, so I filed I-539 to change to H4 and ensured that I am in status. Now employer has filed I-129 (H1B extension) in premium processing and it is approved.

Now, I want to withdraw the H4 Change of Status application. I have receiptNumber for H4.
1. How do I withdraw the H4 application?
2. Is there a form? What information is needed on the withdrawal? does it needs to be notorized ?
3. Do I need to explain why I filed H4?
4. And How would I know if they have received H4 withdrawal letter and agreed to do so?
5. Is it better to have a lawyer or I can do this on my own?

1. How do I withdraw the H4 application? Write a letter with copy of H4 receit through registered mail with return receipt card ( light green card)2. Is there a form? What information is needed on the withdrawal? does it needs to be notorized ?
3. Do I need to explain why I filed H4? YES4. And How would I know if they have received H4 withdrawal letter and agreed to do so? US mail you will get acknowledgement card sign by USCIS rep.5. Is it better to have a lawyer or I can do this on my own? You can do, but if you filed the H4 through an attorney , only attorney has to withdraw the h4 application
