h4 visa application - name in the passport conflict

My fiance's name in her passport has just her first name listed in the "Full name" field.
When applying for her H4 visa, is it ok to include her last name with her first name?

Better to get new passport copy with her First name, Middle name and Last name. Then she should apply for H4 visa stamp so that in future no problems.
