H4 to H1b start working on H1B without getting h1b stamped

Hi,
I am working on H1b visa and I have filed for my wife's h4 which she should easily get in march.

We have a employer willing to file for her h1b in April. However she will not be able to travel on h4 before May-June 15th for some reasons. My question is

1) Can she travel in june(end) on H4 visa once her h1b is filed? If so, once the H1b approval comes, Can she start working on H1b from October without going to India for H1b stamping?

While h1 COS is pending, it is not advisable to travel. If you do so, her H1 may be approved (without I 94 at the bottom) but she can not start working without going out of US and coming back with new i 94 with caveat H1.
