H4 - How long does it take to approve?

Hi,
I am planning to apply H1 and trying to get H4 for my SPOUSE. Right now i am in canada. I am wondering how long does it takes to get H4 approved?

Also do i have to be in US (have paystub with US Employer) to apply for H4 for my spouse?

your spouse can apply H4 visa when you go for H1 visa stamp. US employer only files H1 petition not H4 petition,
