Urgent_ Please help with H4 Documents

I am going on H4. I was on H1 before that.
May I know what are all docs i will need to submit?

General:
1) Form I-539
2) Paycheck of $140
3) Mariage certificate

My own docs:
1) My latest original I-94 ( the one which i received for my H1 approval)
2)Copies of my Passport and Visa

Spouses Docs:
1) Letter from his employer
2) Copy of I-94 (H1)
3) Copy of INS letter of H1 approval


Do my spouse need to make Affidavit of Support. he Just started with his new job and doesn't have his paystub yet?
May be a letter from Bank would Help?

Please suggest if anyhting is missing.

U mean check for $140 right ?
The letter from employer should mention salary of your spouse. A bank statement will not hurt (in addition to salary mention)

Letter from Spouse with copies of passport, visa stamp, H1 document (all certified perferably) - like a cover letter

Then U should write a cover letter.

U should be also be clear on the dates. When first H1 acquired, date of marriage, date of termination etc.
Marriage certificate can be certified copy.
