H1B to H4 Visa Conversion Question

Hello,
I have a quick question about H1b to H4 visa conversion for my wife and I will be very grateful if you can help me get this answered so that I can start the processing and paperwork.

Currently my wife works for State on an H1B visa and I am thinking of getting her on H4 as she is pregnant. I have few questions around that and I will be very grateful if you help me find answers to this questions

1) Do I need to have my H1B visa stamped before she can apply for H4?
2) What all documents are needed for the status conversion (for both)?
3) Does she need to go out of the country to get her H4 or can she apply it from here?
4) If she is pregnant can she convert her status from H1 to H4 or that does not have to do with anything?
5) Her delivery is expected for February 2008 and so when will it be a good time for status conversion.

Any help we be highly appreciated

----------------NO, copy of your valid I-94 or copy of your latest H1 approval is enough
2) What all documents are needed for the status conversion (for both)?
------------ Your H1 approval notice copy, her valid I-94 copy(front and back) your I-94 (front and back) form I-539, fee
3) Does she need to go out of the country to get her H4 or can she apply it from here?
-------------She can apply Change of status from H1 to H4 in US
4) If she is pregnant can she convert her status from H1 to H4 or that does not have to do with anything?

---------------- that She or you have to decide. ask her doctor when she need to stop work, even if she does not apply from H1 to H4 Change of status she can get leave under FMLA
