H1b change of status and visa

Hi to all, may be somebody could help with this question: I am a J1, my DS2019 and passport showed that I was not subject to the 2yr requirement, so in may 2005 I applied for the H1B -premium processing- and I've got the response from USCIS saying that my H1B WAS approved, HOWEVER I had to have the waiver for the 2yr requirement and for that reason my "change of status" was denied. I called them and they told me that I should get the waiver in order to be able to change my status to the H since the H was already approved. So I did, and we just got the waiver approved. I have two questions: Is the change of status automatic or do I need to send more documents to USCIS, (or go to an Inmigrations office)?, or I have to leave the country and get a new visa in order to 'activate' the H.
The second question is, do I have to go to my home country to get the visa or can I just go to Canada (Calgary, since it is close to me) and get the visa over there.
I will appreciate any help on this matter. Thanks!
 
you need to file COS again as you do not have I94 for H1 or you need to get H1 stamp and reeneter.
