H1 revalidation from St Louis

I want to get my h1B revalidated from St Louis.I have my H1 approval notice (I797) extended till 12/2006 with new I-94 but my passport which had the previous H1 visa stamp was stolen. So as of now I have a duplicate passport which has no visa stamp.I want to know what documents will I need to provide for revalidation.I have the copy of stolen passport which has previous H1B stamp and I have the police report.Are these documents enough besides employment letter,DS-156,etc.

------ Send copy of new Passport, Attach all the documents and copy of stolen passport all the pages, attach the copy of police report, you have to fill DS-156 and if male between 16 to 45 then also fill up Form DS-157. Write every info in the cover letter then see what happens. If you are from Muslim country or from a country that commit immigration frauds or from South India under Chennai consulate India or from Gujarat India, I hope they may suspect that some body else may have entered on your passport or you may have sold your passport with US visa stamp. DOS may investigate, if you are lucky you may get H1 from DOS or get it from your home country when you visit because you have H1B approval with valid I-94 you are in status and can work stay till the expire date of I-94, Visa stamp is just for entry to US. You can apply in your home country if any problem with DOS. You will only know once you apply.
