H1 restamp B'bay appointment process

Hello,
I am on H1b visa and am planning to go to India in Dec. I want to take an appointment for re-stamping of H1B. Can someone please explain me the process of how to get an appointment? Since I am here, how do I pay the fees, send the documents, etc.
