H1 Extention, H4, I-94 question

Your responsibility to apply H4 extn. go to www.uscis.gov site for latest fees.
If H1B is extended, will USCIS send a new I-94 for dependents (H4 holders) also? NO Do you have to specifically apply for H4 extension YES or (new I-94)? Who applies for this...is it the Employer or the application? What is the pocedure and how much does it cost?

My lawyer just renewed my husbands H1-B and also renewed the H4 for my daughter and I.
He send me to sign :
Form I-539 Application to extend/change non-immigrant status. (1 form for the both of us, both people sign at bottom of form)
Form G-28 (one each for me and my daughter); Notice of entry of appearance as attorney or representative.
I had to submit also the pasport copies and I-94 copies.
And this is filed with my husband's paperwork in one envelope.
When approved you will get a seperate approval notice for each person, together with a New I-94, that is a perforated card at the bottom of the notice.

Thanks for your replies. Yes, your replies are really helpful. From your replies, I can summerise,
a) An application for extension of H4 also needs to be filed.
b) Who applies (employer or applicant) differs from case to case.
So, I will talk to my employer and find what "who does what".
