H 1B Transfer with extension at the same time?

Hi,

I am currently working with Company A and my H 1B expires in September 2008. I might get an offer with Company B soon. My question is

1) Can I apply for an extension at the same time as the transfer, since the fees are the same?
2) In Part 2. Question 2 of Form I 129, what box should I check?
a) Box "a" New employment (including new employer filing H-1B extension).

OR
b) Box "e" Change of employer

Also, Im not so clear on what kind of documents will be required for the transfer (and?) extension.

Any help will be highly appreciated.

Every transfer is new petition. You can ask for up to 3 years employment.
