Thank you for your quick answers.Do I have to send them the original or a copy of my Green card while filing I-90???

Here is the info.

If your card was issued with incorrect data due to USCIS administrative error, you must mail a new I-90 application using Application Reason "d" My card was issued with incorrect information because of a USCIS administrative error, and any supporting documentation, to the USCIS Service Center/National Benefits Center(NBC) that processed the application/form from which the incorrect card was created. Check the envelope that your I-551 permanent resident card came in to find the mailing address of the Service Center/NBC that mailed you the card. The Service Center/NBC address can also be found on the I-797 "Notice of Action" issued for the application from which the incorrect card was created. In addition to the new I-90 application, applicants are required to send the original I-551 card containing incorrect information, plus the supporting documentation for the requested correction. A replacement application based on a USCIS administrative error must also include an explanation.
There is no fee required. Total filing fee - $0.

I guess you have to use mail instead of E-file. see the attachment in the link.

I guess birth certificate is good enough.
The #6 guy in link listed up what he sent. He sent copy of birth certificate and he got card corrected, but if you have original, it might be safe.
No need AR11. DOn't know about passport copy, but probably not necessary.

Now I have to say this. I-90 takes time. Expect at least a few months.. if you need to travel soon, get I-551 stamp in passport.

God, that must have been so annoying, you wait years and they make a silly mistake and now have all the hassle of fixing yourself

Same thing happened to one of my wife's best friend 15 years ago. She's not the most immigration-savvy person (and she was 9 years old back then), but guess what? While my wife and her friends all became USC after 5 years or so, she is still a GC holder with no citizenship (they were all refugees).
Now I'm convincing her to apply for citizenship explaining the mistake (they inverted day and month of birth, they put May 1 instead of Jan 5).
That would work. Either contact them or just keep your wrong DOB and fix it when you apply for citizenship.

She should have had it corrected 15 years ago  ... has she been using a wrong DOB on everything since then? Marriage certificates, Driver's License, etc?

She tried when she had to renew her GC, but it didn't work, I guess.
She's not married and she doesn't drive, but Social Security has the right DOB.
I don't think it's a big deal. She will just write a letter to USCIS with her N-400 and a copy of her translated birth certificate.
