Form I-693, Medical examination Confusion!!

hello all,
i am about to file for AOS thru marriage , and kinda confused about medical exam. is medical exam requirement waived if married with USC??or do i still have fill out and submit i693 form along with I-485???
it will very helpful if any one could provide me any info or experience reagarding this form.

yes, you will need the medical form with I-485, so do it ASAP to save your time. Call the doctor to make an appointment, the list is on the uscis website. you do not need to print out any form, the doctors will have everything ready.

What is a “medical waiver” and what does it do?
A medical waiver permits an immigration applicant to be allowed into, or remain in the United States despite having a health condition identified as medical grounds of inadmissibility. Terms and conditions can be applied to a medical waiver on a case by case basis.
You are eligible for a waiver if:


You are the spouse or unmarried son or daughter or the minor, unmarried adopted child of a U.S. citizen or LPR; or
You have a son or daughter or lawfully adopted child who is a U.S. citizen or LPR; or
You are eligible for classification as a self-petitioning spouse or child (including your derivative children) because of abuse.
 
my understanding

if during the medical exam, you are found out to have some health problem or you can not receive the vaccinations because of pregnency, you may need to apply for a waiver.
 
I made a few calls to find out about fees to do the medical and some offices charged a fee if you didn't have a form- ridiculous I know, but call ahead and find out exactly what you need to take with you.

Question- I thought the medical exam should be done closer to the interview date? If not does it expire?.... I just did a quick look at the form and answered my own question. I am posting this anyways :-)

Where to File: If filing for adjustment of status at a Service Center, you should submit the completed Form I-693, sealed in an envelope from the physician, with Form I-485 package to the Service Center. If filing for adjustment of status at a local office, you should submit the completed Form I-693, sealed in an envelope from the physician, at the time of your adjustment of status interview. Please note: some local offices request that you file Form I-693 with your adjustment of status application packet to allow for quicker processing. Please check the web page of your local office under "Local Filing Procedures" to determine if your local office wants Form I-693 at the time of filing Form I-485 or at the time of interview.

When Applicants Should Get Their Medical Exam: The results of the medical examination are generally valid for only 12 months. Applicants should schedule the medical examination as close as possible to the time you file for Adjustment of Status.

Sheracar, for your "where to file" read the entire instructions at USCIS for AOS family based. If you are family based of immediate relative of a USC and applying for AOS, you MUST submitt all forms and evidence (including medical unless you want a RFE) to Chicago Lockbox. This is pretty well explain at the USCIS website and in many posts over here.

It is recommended to get your medical done closer to the filing of the forms since they are valid for around 12 months from the date of the medical is done, but sometimes depends if you have all your vaccinations on time or if you need a treatment for something. There are a part of the form I-693 that you can download at the site, but the rest of the forms your doctor anyway will have it. For your records ask your doctor to give you a copy and do not open the sealed envelope.

livelife, you must read ALL the instructions not in parts and you will understand. Actually you must go to the doctor anyway even if you are pregnant and the doctor is the one who determine that you file a waiver in case that you never get the vaccinations that are required because of your pregnancy in that case. Later on, after your pregnancy is over then probably you will have to undergo to the other part.

That boat has sailed for me awhile now. I noticed that everyone has advised to submit the medical form along with the other documents and I had remembered that I did not do the same at the time of filiing (2004). I did receive a notice to do a medical and to submit at the time of the interview. A lot has probably changed since I filed so... www.uscis.gov

ahhhhhmmm what's an RFE???

I paid the same, including two shots, I had MMR already so I do not need that shot.

I called at least 6 and I found one that charge $100+ (don't remember exact but it was not over $150. Are you in NY? Call a few!!
 
My wife just finished lately.
Highly recommend to make an appointment right after you decide to apply for green card. Because the appointment takes very long time. My wife called around at least 10. She got lucky because one had cancellation so she could do it in a week after called. Otherwise next available date is April 15th almost everywhere.

She found one (university medical center) charge $100 for filing, either one $50 for x-ray or $100 for T-B test. (civil surgeon recommend T-B test though.)
So she totally paid $150 for the exam. (filing + X-ray)

X-ray, you will get it within 10 minutes.
T-B test takes about 3 days for result, most important, you need to go back to the center for result. So it's kinda money and time-consuming.
Blood test takes about 3 days for result. But you don't need to go back there, they will send the form in a sealed envelop to your current address. You just need to put that envelop in your package.

This is in Minneapolis, Minnesota.

BTW, civil surgeon will ask you to bring your passport, I-94, and record of shots(very important! you want to get this ready before the filing, it really helps a lot.)

Yo don't necessarily need to have an X-ray done, I think you only have to get it if you get a skin reaction to something... I had to get it so it was $100 more. The doctor said that many foreign people get a reaction because we get a shot against (whatever it is) so our body already has antibodies for it.

i called 6 docotrs and least the doctor asked was 190 + the money for tests
do u guys know how it costs for medical tests in lab?

1 of doctors was asking 400 all inclusive do u think this is better
or to go for doctor with 190 $
please let me know

I would say that you should keep looking around. I paid $200 for all the tests, plus $100 for the X-ray. The tests should already be included in those $190. Call more doctors
 
I paid $265 for everything including shots.
After that the nurse took all the forms, put it in an envelope and sealed it like crazy. I asked jokingly if I wasn't allowed to see my own results and the nurse said, all my results were fine.
But the only thing I got was the supplementary form about my shot history. Is that normal?

The 5th page of the form I-693 is applicant's copy. They should have given you that page without your asking.
 
Hmmmm... I better give them a call. You see, they had their own CIS-forms and didn't use the one I had. It was completely the same though. Uh-oh! :confused:
Oh well, I'm sure they can photocopy it for me. Or do you have to have that 5th page?

It cost us $250 to get the medical done for my spouse - this included 2 shots + blood test + TB Test, etc. Since the skin test was negative, there was no need for an X-Ray. Had he needed an X-Ray that would have come at an additional $60. We called several docs and the first one asked for $550, so it was a big relief when we heard $250. Later we also found one that charged only $185... but that's alright. I think its best to call a few in your area before making an appointment

He (my husband) was not required to bring the form I-693 (the doc had it) or any document like I-94, etc. He remembered his passport no and provided that to the doc.

There are three or uhmm four copies of the 693 form, one for civil surgeon, one for the USCIS, one for.... and one for the patient. You will get the patient's copy, however, no one knows if the sealed envelope contains more than what's in the patient's copy. I believe that's 'FOR USCIS EYES ONLY' :rolleyes:
