Form I 134

As far as I know every winner of the dv lottery should submit an I 134 form for affidavite and support in his interview day tot he officer. I have some questions regarding this procedure that
-who should fill out this form?
-this form is downloaded fom the internet and which site?
- if someone doesnt have anybody in US, then how much dollars should he should has in bank to show his bank statement? ( for three person)
- is it possible that someone who is not permenant residence in the usa right now, could fill this form? I mean someone who has not got his green card but he is staying in america and working there?
 
Thanks alot dear, but one question more. One of my friend is living at USA since six months, and he hasnt got his green card he is waiting for his green card. Could he fill a i 134 to me or that person should be a us citizen?

I came in US two years ago, I was selected for DV2014 (low CN).
I am from EU region and didn't need to provide any proof to embassy (no money or form I 134 or any other proof).
My parents have been selected last year DV2015 and their number is current in september. They are about 50 years old and I'm afraid that embassy will require some proof for them (as you know it's a last month and I don't want them to loose chance if they ask for proof and we get out of time).
What do you think, should I provide I-134 or they will be fine?

(Not simon but) if you can provide a I134 why not do so? It can't hurt but could help a lot.

He may not have good proof of income as he is so recently arrived. ..

I have one more question about this.
Do I need to put household income or only my personal income when I'm filling this?
I don't have any big savings in bank or any other things except income as a proof.

Your total income as shown on the W-2/1099 (whichever is applicable) that you will be providing with the I-134.
