My interview date will be in Dec/Sep. when should I request from my sponser to fill & send the I-134 for me? I have heard that this form should not be more than 2 months from the time I using them. Is it right? Please help me.

I don't know how old the affidavit can be.

actually, the affidavit is good for 6 months, and must be valid at the time of your interview. So, if less than 6 months has elapsed since it was signed, you should be able to use it.
