Go to INS site...
Download the forms. Fill it up (c09 is the answer for the last question). Take 2 ADIT style photos. Attach copy of I485 receipt. Write A# and receipt number on all documents other than the I765 application.
Find from INS site (depending on service center ) where to send EAD (and AP), generally to the Service center - but PO box will be different for EAD and AP.
Cut a check for $120 favor of INS- and send it. Send it with some kind of tracking. Watch for your checks to get cashed and wait for your receipt in the mail.

If you think you need the signature card, you can go to the local INS office and get one, applications can be sent without the signature card too...

Find the Alien number in I485 receipt.
I94 number is for other type of applicants (F1-OPT etc.). For I485 AOS applicants you need Alien Number.
If you do not have one, call INS during office hours and quote your 485 receipt number to find your A#
A# is required for EAD.

applying EAD. which i-94 to be attached? expried or new new i-797? please reply.

I am planning to apply EAD myself.
read in EAD instruction booklet that we need to attach i-94. got expired in passport i-94 and new i-797(new h1 approval notice,
this includes new i-94 at bottom).
which one i should send? or both.
please write exactly which ones to send based on your personal experiences , who got approvals.
ALSO should I write both A number as well H1 number in column 10 in EAD application? or just A number is sufficient?
got new h1 stamping in passport too from st.louis, should i send the copy of that too?
