The address to send the application, fees, documents required to be attached with it, everything is pointed out clearly in the package of EAD application. You can download it from any immigration related website. For your reference it is Form I-765. As far I can remember the documents you need to attach with your application form I-765 are:
1. I-485 receipt notice
2. A copy of I-94 Arrival/Departure Record (if available)
3. A copy of your last EAD
4. 2 photos (immigration style)
5. Fee of $100.00 (Check or money order must be payable to "Immigration and naturalization Services"
6. Form I-765 Signature Card (A green small card). This one you probably will not be able to down load from website. You have to go to your local INS office/lawyer\'s office or you may request for this over telephone to INS to mail it to your address.
The address to send everything depends on your application. If your response to question 16 of the form I-765 is c(9), which is most likely be, then you will have to file your application at the same local INS office or Service Center where you submitted your adjustment application.
Yes, I went through my lawyer. But there is no reason that you can not do it by yourself. It\'s a a very simple and straight forward form. I payed extra $100.00 to my lawyer just to have him as my back up in case of any mishap. Infact, I prepared everything by myself. The lawyer only signed it. One last thing, if you decide to go through your lawyer, you will have to fill up another form, called G-28.
