Again. The message means that the second attempt was not successfully submitted so it is not regarded as a duplicate entry. There is no reason for disqualification.

Did someone do it and win? The number of people who do that is tiny and the chance of winning is something like 0.5%-1% in a normal year. So maybe they did but the chances we hear about it on this forum are pretty small.

Yes
Sometimes it can also mean 2 confirmation numbers for the same person where the entered information may be slightly different in an attempt to stop the system picking up the duplicate
