I'm not sure if this is in the police certificate, but my mum was summoned to appear in court for failing to send in her tax form (I know, this is stupid, cos she paid the taxes, but didn't send the form). She was fined.
Does she have to include this in her sworn statement under the question "Have you ever been charged, arrested or convicted of any offense or crime?"
(Question 31 on DS-230 Part II).
