DS 156 Questions

Hi All,

I have a couple of questions on the DS 156 forms :
Q 30 : Have you ever been issued a US visa?
Since I have been issued two US visas in the past - one F1 and one H1-B, which one do they expect? I'm assuming they mean the very first one I ever received. Can anybody tell me what responses have worked at Chennai in the past?

Q 15 : Home address
A cheat-sheet for DS 156 from my company's lawyers says that I must use my US home address. But it also says that individual US consulates may expect a local address.
Can anybody who's had a successful interview at Chennai tell me what they put down?

----- If applying Visa revalidation IN US with DOS then provide US home address, IF applying Chennai Consulate then India Home address

I know that I have to answer "yes" to 1. - I was just wondering which visa's details (when, where) they expect.

Also, if I need to give Indian Home address and Phone numbers, should I put in US phone numbers for Business phone and business fax questions?

----- if you are confused then write on cover letter please find SHEET A for item # and attach the SHEET A write Item # and explain in detail

----- just give your India home address and phone # as you are in India you dont have buisness # the Form is for all kind of Visa so all the items may not be applicable to you
